Sliding Contribution Rates
In order to provide services to as many individuals and families as possible, I request your active participation in deciding your level of contribution for my services.
Please bear in mind that we are socialized to see what we lack better than the big and small privileges we do have.
Those able to pay on the higher end of the scale help me serve those on the lower end of the scale.
I am open to questions about this fee structure and look forward to serving you.

Please read the 3 situations below. Depending on your CURRENT situation, you'll be able to choose your level of contribution.
Your patient intake form will have a place where you will be able to input your assessment at that time.
(This sliding scale and some verbiage were introduced by the Andean Medicine for Liberation Workshop via PDDC. Nelhuayo appreciates the intellectual labor in creating this useful tool. If you know the original creator, we’d love to create and compensate them.)
